When someone has been taken through homicide or a culpable road death, families can be left facing shock, grief, confusion, and practical challenges all at once.

At The Homicide Support Hub, we understand that every family’s situation is different. Some people may need emotional support. Others may need practical guidance, help understanding what support is available, or simply someone who will listen without judgement.

That is why we use the F.A.S.H Form.

What does F.A.S.H stand for?

F.A.S.H stands for Family Assessment Support Homicide.

The form has been created to help us better understand your circumstances, your concerns, and any additional needs you or your family members may have.

Why do we ask you to complete the F.A.S.H Form?

After a homicide or culpable road death, it is not always easy to explain what you need. You may not know what support exists, what questions to ask, or where to begin.

The F.A.S.H Form gives you a safe way to share important information with The Hub, so we can understand your situation more clearly and offer support that is relevant to you.

It is not about judging your circumstances or putting you through a complicated process. It is about listening properly and making sure we have the right information to help you and your family as best we can.

How can The Hub support you?

The Homicide Support Hub provides peer support and practical advice for families affected by homicide and culpable road deaths.

Once we understand your situation, we can help point you towards trusted support that fits your needs. This may include emotional support, peer support from people with lived experience, practical guidance, or signposting to other relevant services.

Every family is different, so the support offered should never feel one-size-fits-all. The F.A.S.H Form helps us understand what matters most to you.

What happens after you complete the form?

Once you complete the F.A.S.H Form, The Hub will review the information you have shared. From there, we will listen to your needs and provide the help that is most relevant to you and your family members.

You do not need to have everything worked out before reaching out. The form is simply a first step towards helping us understand how we may be able to support you.
