Whether you’re seeking help for yourself or referring someone else, the best way to begin is by completing a F.A.S.H (Family Assessment Support Homicide) form. This allows us to understand the person’s circumstances, identify immediate concerns, and begin offering tailored support straight away.
- For yourself or a family member: Complete the Self-Referral F.A.S.H Form
- To refer someone else (as a professional, friend, or associate): Complete the Organisation Referral F.A.S.H Form
What Happens After a Referral?
Once we receive a completed F.A.S.H form, a member of our team will carefully review the information. We respond with empathy and care, ensuring that the support we offer is appropriate to the individual’s needs.
Help might include emotional guidance, peer support, or assistance navigating the criminal justice system. Where necessary, we can also refer individuals to trusted specialist services such as bereavement counsellors, trauma therapists, or cultural support groups.
